Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

[review] => On the 4th of April 2024, my mother had a true emergency where she was unconscious and not breathing. The situation was handle in a Professional Manner. She got Immediate help , she started breathing . Called me her daughter Deborah Connelly, and Medical Advocate. I am also, a Nurse with vast knowledge and experience. The communication was, precisely communicated so I could make decisions to send her to most experience Hospital for her condition. I would like to give special thanks and kudos to Amber RNP, Dee Pct for their help in the situation. Elizabeth LPN and Melissa LPN for going above and beyond . Staff sometimes does not get the appreciation they deserve. I would like to let you all know, Bret Brown Administer and Nicole Rn DON, has always given me a lending ear to hear my concerns. Staff feels like family.Thank you Bonifay Rehab and Nursing Center.

I am beyond overwhelmed with the kindness, compassion and care that the staff has showed my Mother and my family. Everyone is so friendly and willing to assist with my Mothers care and it shows with the difference in my Mother. She tells me almost everyday that she loves her new home and she is so happy. It makes such a big difference when you see your loved one cared for appropriately and it lets me sleep better knowing that I don’t have to worry that she’s not getting the care she needs and deserves.

[review] => This is the most caring, kind, compassionate team I've ever dealt with. My mother moved here after being in long term care. We were unsure of what to expect but the team of nurses, aids, and therapist were outstanding. They worked diligently with mother to improve her quality of life. When the time came for her to transition, the staff was so considerate. They moved her roommate out. Brought in a extra chairs for us and a hospitality cart with drinks and food so we could be with her around the clock until her time came. They were wonderful to us. I would highly recommend this facility.

Wish you would have told us earlier. You had all his meds before today. Now we have to go through this process again. Imo not a great way to run things. I understand policies but you knew what he was taking….someone dropped the ball. Anyway best wishes for your faculty in the future. Hopefully this doesn’t happen to someone else.

[review] => I have been a resident for over 7 years. I came here when I was 49 years old and came from a active life. It was very difficult for me to adjust to this life. I have a small room with a roommate and limited privacy. We do not have options for recreational activity transportation. But we do go to walmart weekly. NOT anymore, covid has caused transportation difficultis, Medical appointments are not a issue. This facility is very clean and well maintained. The kitchen staff trys hard to fill my very picky appetite. The BEST part is the staff. From the admin to maintenance to the skilled, caring nursing here is outstanding. Never in my life have I felt more loved or cared for. I feel blessed to be here. This was written in 2019. Updated, covid has created challeges for everyone on the planet, this nursing home as well. MGMT has done thier best, same MGR from earlier review. They try to maintain a high level of care with a few less employees than before covid, To me, they mostly succeed. I am still blessed to be here now over 7 years.

[review] => My husband was in rehab at Bonifay Rehab. He was supposed to be in rehab, like have physical therapy and all of that, but he fell twice. He was only there briefly and nobody was really watching him in the middle of the night. They didn't have alarms in their beds, so my husband climbed up and fell again on the floor naked, crawling to his roommate. Then later, he fell again in the bathroom and had a seizure, that's why he went to the emergency. They're very negligent. I will never put my husband back there, and I don't recommend them. It's just too much. I'm so sorry to say that because I know someone who works there and she is my neighbor, which is the reason why I put him there. However, in his short time being there he fell twice; that's negligence. I saw their negligence right away. I didn't really see how they work.
I went there, but I couldn't get in and follow my husband inside the room. I was not allowed to get in. Everything was sad. They needed to really change the approach of this type of business. I didn't see any staff when I was there, then I had to leave because they wouldn't let me in. Then the next day, they called me and told me that my husband fell on the floor and they took care of him and everything. I went and visited him, but I cannot get in. Then the next day he fell in the bathroom and had a seizure, so they had to bring him to the emergency room.

She made us feel very at ease about the care our grandfather would receive at the facility. Unfortunately, our Papa passed before we were able to make our decision. [review] => My Mom is in this Facility. She is 85 yrs old. She has been dropped many times on the floor. She fell out of her chair cuz the didn’t have seat belt on. No one was watching her. She hit the floor face first and split her eye brow wide open. Had to go to the hospital for stitches. This last episode, she went back to the hospital, dehydrated and with a UTI infection cuz they make her sit in her poo. I wouldn’t send my dog to this facility. Jus my opinion

[review] => Fantastic nursing home. Staff treats my dad very well. He loves the staff as much as they do him. Very clean nursing home. Big large rooms for residences to share. They stay on top of their residence care. My father has been taken great care of since he been here. [review] => Absolutely the best decision I ever made was to move my mother to this facility. Prior to coming to Bonifay Nursing and Rehab, my mother was in an excellent, top-rated facility in Pensacola. They seemed to be taking great care of her, but she was basically an invalid. Each time I visited, she was in her bed and mainly slept. NOT at Bonifay!! My mother immediately began to show signs of progression, not only in her mental health but in her physical health as well after entering their center. After about a month at Bonifay, I walked in one day to find her sitting in the hall, looking better than I had seen her look in over two years. It was like we had her back again! Her mental health is strong, her outlook bright, and she no longer just lies in bed. They get her up each and every day, and she is out and about with their other patients, living life to the fullest. I cannot say enough about this wonderful facility that gave my mother back to us again! FIVE STARS all the way around. They truly care about patients and families at this facility!

Bonifay Nursing and Rehabilitation Center is a place I’d happily recommend for your loved ones. The staff is amazing very friendly, attentive and compassionate. The director of nursing Nicole is a godsend she has such a caring heart and kind spirit. The facility is clean, and inviting. The residents are happy and thriving.
